Q: Who is eligible to receive the incentive?
A: Per NCA 21-104 passed by the Muscogee (Creek) National Council on August 28, 2021, all Muscogee Nation citizens ages 12 years or older are eligible for an incentive. Citizens who are tribal employees/contract works, including the College of the Muscogee Nation, Gaming, and Health employees will receive the incentive through their employer.
Q: How much is the incentive?
A: All eligible citizens who are fully vaccinated by September 30, 2021 will receive $500. Anyone else who is eligible and fully vaccinated by December 31, 2021 will receive $300.
Q: What if I’m only partially vaccinated?
A: According to the law, you must be fully vaccinated to receive the incentive by December 31, 2021.
Q: What do I need to do to apply for the incentive?
A: Starting at 10:00 am on Friday, October 1, you may complete the Vaccine Incentive Application on the Camphouse portal. Your information will be stored on the site if you have previously registered.
Q: Do I need a copy of my vaccination card?
A: Yes, during the application process you will be asked to upload a scanned copy of your vaccination record. If you do not have a copy, please contact the provider where you were vaccinated.
Q: What information is required to apply?
A: Citizens must provide their social security number, MCN Roll number, date of birth, physical (and mailing) address, and list of MCN citizen household members who have been fully vaccinated. You will also need to provide detailed vaccination information including a copy of vaccination record, type of vaccination, and dates of doses.
Q: How will I receive payment?
A: If you previously received ARPA Direct Assistance, the incentive will be automatically added to your Netspend card that you already have received. If you have not received ARPA Direct Assistance yet, you will be issued a Mastercard debit card.
Q: When can I expect to receive my payment?
A: We anticipate 10-14 days for application processing and approval. Please allow an additional 10 days after the approval to receive your money. The status of your application may be checked in the Camphouse Portal under “My Applications.”
